You agree to provide complete and accurate medical information and to update us promptly if any of this information changes.
Confidentiality
Personal Information and Privacy:
- We must collect personal information relevant to your condition
- This information is necessary for your assessment and treatment
- Not sharing information may limit the services we can provide
Your information remains confidential and secure except when:
By law if a court subpoenas your record the clinic and osteopath must comply.
By law there is a duty to report if we believe you or someone else is in danger; This includes elderly and children.
- Writing reports for other professionals
- For Golf Assessment clients, we can share relevant findings with your golf coach with your explicit permission.
- Discussing your case with family or employers
- Law requires disclosure
- Claims require summary reports to doctors or agencies
Understanding Your Treatment
- Your diagnosis and what it means

- The recommended treatment plan
- Possible risks and side effects
- You can stop treatment at any time
- Your diagnosis may change as we see how your body responds
- We DO NOT perform “cracking” techniques (High-Velocity Low-Amplitude/HVLA)

- I will check in with you every step of the way to ensure you are not in pain.
You may feel some soreness for 24-72 hours after treatment

Some discomfort during examination is normal as we identify problem areas

Osteopathic Consultation and Treatment Fees Our fee schedule is as follows:
Fees and Payment:

Initial Consultation (90 minutes) – $220

Golf Assessment Fees
- Initial Golf Assessment (90 minutes): $220
- Follow-up Session (45 minutes): $120
- Golf Assessment Package (Initial + Follow-up): $315
- Cancellations: Less than 48 hours’ notice – any less than 24hours notice full fee applies
- Payment is required after the appointment, unless payment is made online in advance.
Cancellation Policy:
If you need to cancel or postpone your appointment, please provide at least 48 hours’ notice; otherwise, you will be charged according to our schedule of fees listed above. When you make an appointment, the entire appointment is reserved for you.
- Less than 48 hours’ notice – 50% fee applies
- Less than 24 hours’ notice – Full fee applies
- Online bookings require payment at the time of booking
- Late arrivals cannot be extended
- Three missed appointments may result in no future bookings
- Appointment reminders are courtesy only – you are responsible for remembering appointments
- While extenuating situations are taken into account, there is an expectation that we consider your time as valuable as you should consider ours.
Cancellation fees apply because your session cannot be offered to anyone else without sufficient notice. Unfortunately, if you are late for your appointment, your Osteopath generally cannot extend it. This ensures that the Osteopath remains on time for the next client. Please note that Medicare will not provide a rebate for cancellations. If you fail to attend three appointments without prior consent, True Health Osteopathy reserves the right not to schedule further appointments.
While we endeavour to remind you of your appointments via email, you should not rely solely on these reminders, and it is your responsibility to remember the appointments you have made. Cancellation fees will be charged using the credit card details stored in our secure payment system, Stripe.
Full payment required: True Health Osteopathy reserves the right to refuse further appointments for clients with unpaid invoices. Fees are charged at the end of your appointment. Payment is required at the end of the appointment.
- For online bookings, full payment is required at the time of booking
- We accept credit cards and debit cards (preferred)
- Cash payments need the exact amount
- Unpaid invoices may result in the refusal of future appointments
- Refunds will be processed back to the original payment card, minus any applicable processing fees. Please note that Stripe payments charge a processing fee, which will not be refunded if your appointment is cancelled.
- All refunds will be processed within 5-10 business days
Medicare Care Plan Information:
- If you are eligible for Medicare rebates, this practice can claim them on your behalf using its online Medicare rebates claiming service. We will not offer on-the-spot rebates as the time involved affects the osteopath’s ability to stay on time.
- Eligible Medicare rebates: $60.35 (as of July 2024)
- The clinic must receive referrals prior to the appointment; if supplied at the time of the appointment, they will not be used until the next scheduled appointment.
- Minimum of one to the maximum of five appointments per calendar year with GP Care Plan
- Full payment is required upfront
- Rebates processed online within 72 hours
You must ensure valid referrals and track session numbers
Private Health Insurance:
- Check your coverage level with your fund
- Rebate amounts depend on your policy
- Detailed receipts provided for claims
- HICAPS available for immediate processing with a health fund card
Important Notes:
- We are not a bulk billing clinic
- You are responsible for all fees
- We do not provide emergency services

Recording and Technology Consent
We may record your gait or exercises for clinical documentation
Recording is done through the OnForm App (www.onform.com/privacy)
Recording will never be done without your knowledge
You must give consent before any recording
You can:
Refuse recording at any time
Ask for footage to be deleted
Withdraw consent at any stage
We use an AI medical scribe called Heidi to document consultations
